The following questionnaire is essential information for me to ensure your safety.
Please read the questions and select YES or NO in the required fields. If the answer to any question is YES, Things Change will need to discuss your condition and decide whether it is safe for you and others to attend the class.
Failure to not answer the questionnaire or to provide incorrect information before breathwork and cold therapy may endanger your health and the safety of others.
Note: Answering a question with YES will not necessarily prevent you from participating in a class with Things Change, but you may need to seek medical advice and guidance prior to breathwork and cold therapy.
